NOT "COOL": Your dad's doctor should be consulted ASAP. It is his/her DUTY if a safety issue is involved to counsel your dad to give up his license or report the situation to the local State Authority (in MA. for example it would be the Registry of Motor Vehicles. BE AWARE SENIORS DO NOT WISH TO GIVE UP THEIR LICENSES IT IS A MATTER OF PRIDE AND DESIRE TO BE "INDEPENDENT" !! HANDLE WITH CARE!

Extremely serious: Even in your brief description, you mention 2 worrisome features: size and proximity to the aorta.  It's beyond the scope of this forum to speculate on his prognosis, but i think you're aware that your father is in trouble. Talk with his oncologist. There are several types of lung cancer and some are more sensitive to chemotherapy or radiation. Best of luck.

This : This sharp pain on the left side of chest could be from your lungs, heart or mediastinum.  It is possible that it is lung cancer or pulmonary embolism.  I would reccommend that you get lab work of d dimer, and chest xray to start.  If there is either abnormal, then a ct scan of chest with contrast.  If the chest pain were dull, i would reccommend a stress test and echo cardiogram.  Sharp pains may more likley be from lung or pleura.

Lung: Officially it is a condition of the blood vessels of the lung. The ultimate result of untreated disease however is right sided heart failure.

What kind of hernia?: It depends on the location of the hernia and what kind of surgery was done. Surgery for a hiatal hernia or diaphragmatic hernia may cause scar near the heart. But even this should not lead to a mass between the heart and lung.

: Base on your age, your friend is probably an octogenarian as well. Fluid in the lungs is the lay term for congestive heart failure and with your friend it is most likely due to poor cardiac output from arrhythmia. Ablation sometimes has serious complications which include heart block, acute pericardial effusion with or without cardiac tamponade, etc.
