i am experiencing arrythmia this evening when i lie down. should i be concerned as i am already taking a beta blocker?

Heart rate: Whether or not a heart rate is concerning depends on the clinical context - is there associated shortness of breath?  Chest pain?  Palpitations?  Is it lasting all day or coming and going?  How high does it go?  The elevated heart rate may not have anything to do with the beta blocker not working - lots of things cause high heart rate like caffeine, anxiety, infection to name a few.

May need to stop it: If you've been taking it for just a short time, you can stop the medicine.  If you've been on it for a while, you'll need to taper the dose down .

Best 5mg: Probably best to reduce and then stop but the dose that your taking is probably safe to stop since plain Propranolol (not extended release) is only effective for 4-8 hrs after taken.

Don't do it: Josh, prolong (http://performancenutrition.Herbalife.Com/en/products/prolong) contains 16 grams of sugar per portion. The main ingredient is fructose (sugar). 1 portion is 250 calories. Beyond that, it's just a multivitamin. There's no harm taking it with metoprolol but you're wasting your money and just adding empty calories with little nutritional value.

is it typical to be required to stop beta blocker prior to ep study? i've been taking propranolol for 5 years, 3 x daily. how can i do this?

Yes; taper off / 1wk: Beta blockers can suppress arrhythmias, &amp; the EP study is a provocative test (trying to induce an arrhythmia to see where it's coming from). Stopping beta blockers abruptly can cause "rebound" increases in blood pressure, so the usual method is to taper off. Talk to the EP doc who scheduled the EP study to determine how the taper should be done. (I usually say 1/2 dose every 3 days, but that's me)

Propranolol: A 2sec. pain such as you describe is highly unlikely to have any relation to a medication.

Not a diuretic: It's not a diuretic. If you're incontinent on bisoprolol, you'll be incontinent off bisoprolol.

No: Altace is an ace inhibitor.  Ace inhibitors act differently and on different targets than beta blockers.
