Back surgery: You appear to be very highly motivated and cognizant of your medication use. I would probably stay with a Physiatrist or an Anesthesiologist who does Pain Medicine

Not uncommon: Twenty two  years post pelvic surgery with the only complaint of some pain controlled by mild oral meds is fairly good.  When pelvic surgery and RT used, one thinks of hysterectomy  or rectal Ca for cancer.  The intestine usually drops into the pelvis and RT causes fibrosis around bowel and bladder to produce discomfort.  At 90 no other approach should be used for management.

Be sure of diagnosis: Unless you are an overhead athlete ( pitcher or tennis, etc.) full - thickness rotator cuff tears in your age are very unusual. More commonly seen are shoulders w/ a component of instability and secondary rotator cuff tendinitis/partial thickness injuries and/or biceps anchor injuries'SLAP' tears . See an experienced ORS w/ subspecialty training in shoulder / sports before you have surgery . GL!

Discuss w/ surgeon: Be sure to get detailed directions from your surgery team about this important issue. Be sure you understand it well and ask for instructions for contact help if you discover further questions or problems. You are wise to want to plan ahead. They are the best source of guidance as they should know both your Hx and Tx most completely.

i've had endometriosis & fibroid cysts. had surgery but still have bad cycles. is a muscle relaxer or pain med such as percocet or tramadol better?

None of those: muscle relaxers may or may not give any relief and percocet and tramadol not meant for chronic use due to being addictive.  Lupron (leuprolide) would be a better alternate.  Also its fibroids in the uterus and fibrocystic changes in the breast.  You likely have fibroids (benign tumors of the uterus).  Surgery may help as well.

If : If you are having trouble with your post-surgery pain management and medications, your best bet is to see a doctor who specializes in these problems.  A pain specialist is a doctor who has specialized in either anesthesia, neurology or psychiatry and has concentrated their studies on the management of acute and chronic pain.  Call your primary care doctor for a referral to someone they know and trust.  A pain doctor will know how to navigate making you as comfortable as possible while minimizing side effects.
