Many causes: Indicates that ca is high and that PTH is not the cause.  Other causes include:
too much vitamin d
malignancy
chronic granulomatous disease
thiazides
hyperthyroid

rarely: acromegaly, addison's, milk alkali syndrome, and a few others.

Secondary HPT: Hi. What you describe is called secondary hyperparathyroidism (2o-HPT), and is a physiologic response of the parathyroids to the threat (or presence) of low blood calcium. You need to find out WHY your blood calcium is low or at risk of going low. Treat the underlying problem. Low vit. D is most common but it can also be GI or kidney problem. 2o-HPT pulls calcium out of bone - bad for bone health.

PTH: Low vitamin d or a diet low in calcium can cause PTH elevation in the setting of normal calcium.  Also kidney problems can cause a similar process due to difficulties processing vitamin d.

Check Vitamin D: Low Vitamin D level ( 25 OH Vit .D) is a common cause of high PTH level while Calcium level is Normal. A condition called Secondary Hyperparathyroidism.
Check 25 OH Vitamin D &amp; Ionized Calcium, If ionized Calcium is NL &amp; Vitamin D is less than 30 start Vitamin d treatment and that should correct the PTH level.

High PTH: High PTH can be caused by a parathyroid gland tumor, a genetic condition called fhh (familial hypocalciuric hypercalcemia or low urinary calcium with high blood calcium), and vitamin d deficiency.  While fhh is usually asymptomatic,  high calcium from a parathyroid tumor, or vitamin d deficiency can cause many different symptoms.  An endocrinologist should be able to help find the cause.

I would be most: suspicious of the low vitamin D level. The PTH (normal range considered to be 10-65 pg/ml or ng/L) seems to be about right for the modestly elevated initial calcium level. But if that calcium level was really the result of dehydration (which could be the explanation) then a PTH of 14 is a little low and hypoparathyroidism might be considered.Consider further investigation and vitamin D supplement

him is 55 has ben on dialysis for 5 yrs he has high level of pth 240 for 3 months and he is taking alpha calcitonin daily bt still not decreasing why?

Common problem: in renal failure patients  as Vit D fail to convert to active form leading to secondary hyperparathyroidism . This condition is manageable with restriction of phosphate diets , active form of Vit D . Renal transplantation will reverse most of the time, speak to his doctor
