On the other hand, even normal people can display these symptoms under very stressful circumstances.

Get help: This a difficult problem. Bring up the issue with the patient. If that doesn't work enlist the help of anyone that would be appropriate (patients family and friends).

Possibilities: This is often a feature found in kids with the problem called stool retention.They had  pain on pooping at some point with a hard or bulky stool. They try to avoid going and weaken the anus over time, and begin leaking when they no longer feel it. I would seek out a pedi or pedi gastroenterologist with experience dealing with soilers.It can be fixed but often takes up to a year of hard work.

No: "always" is never true in human behavior. Many people self-harm in private, and do not show their wounds to others. They may even hide them from their psychiatrists and therapists.

No not at all: I think they are trying to get support and overcome a serious problem. Be supportive and encourage them to talk about their challenges. Help them in situations where food may be tempting them to eat improperly.

Cluster b: All disorders within cluster b have social and interpersonal relationship conflicts as a part of the disorder.  To say that one is less or more attention-seeking is not really a symptom, but more an individual difference.  In my experience i've seen bpd patients who have been attention seeking (desperately so) and attention avoidant.

my 4 year old daughter dry heaves and is sometimes sick every time she's upset or crying, is this attention seeking or could it be a serious matter?

Likely behhavioral: A kid may vomit, soil his/her pants, bang his/her head, etc. , if they have not properly transitioned into appropriate methods of reasoning when they develop useful language. Usually triggered when frustrated, mad or not getting their way, this is a behavioral disturbance that may require the help of a therapist to get back on track.

Attention seeking?: I imagine this depends on the person making such a statement, as it implies a judgment rather than an assessment of facts.  People talk with me about their mental illnesses all the time, and i don't call it "attention seeking."  however, i'm a psychiatrist, and this is why they come to me.  If someone is telling about their struggles in social or other situations, others may think what they think.
