: Anal pain accompanied by bleeding, should always be examined by a DR in person. There are a variety of conditions that can cause these symptoms. Anal fissures are just one. Others include hemmorhoids (internal or external), colitis, even cancer.

Pain, constipation: Excruciating pain with bowel movement is the most common symptom.  Pain will also last for several hours after the bowel movement.  In severe cases the patient is afraid to defecate because the act will precipitate pain.  Constipation and narrow stool are also the result of pain and spasm of the internal sphincter muscle.  Bleeding usually minimal.

Low grade fever...: ... is not compatible with the diagnosis of IBS, but suggests either your symptoms need re-evaluation and/or something else is superimposed on your underlying irritable gut. Wait for appointment: Conservative treatment is usually preferred for these issues as there is no guarantee that surgery will make everything rosy.  I'd wait to see what your GI doctor says on Weds.

Have it checked: Having someone lay eyes on the tissue and perhaps sampling to send to a lab is the best way.  And ... often fissures are caused by stool that is too hard and large to pass smoothly.  Eat a good diet high in vegetables (non starch) and fermented foods like real (not make with vinegaer) sauerkrat and pickles.  Adding fiber from food and bacteria like Lactobacillus may help.

i had anal fissure -lis surg on tues. after 1 very tough bm yest and one semi tough today. pain is bad just like fissure symptoms. did surg not work?

Too early: You ate only 3 days after surgery. Healing takes time. Use warm bath soaks & avoid constipation. Follow up with your surgeon soon about the healing.

Pain with BM: Painful bm with or without blood in stool. It would take a doctor to see the actual cut in the rectum to diagnose it.
