Asphyxia: Labor it is a traumatic event for the fetus. Sometimes during labor ombilical cord gets compressed limiting the blood that gets to the baby. Sometimes during labor the blood supply to baby is reduced  even more due to abnormal placenta.  Asphyxia in labor means bsically baby is not receiving enough oxygen to maintain vital functions in optimal condition.

There are several: There are several possibilities for pain during labor. There are local anesthetics like xylocaine, similar to what dentists use to work on your teeth; heavy duty nsaids, demerol, (meperidine hydrochloride) etc. If you have a specific question about a particular med please find out which one.

Yes, a little bit...: Interestingly, this is one of the issues pregnant women most fear.  I field more questions about this as I do about pain control.  Remember, delivering women are pushing into their pelvis (an area that contains bladder and rectum) with more force than they have ever generated.  Baby's head further pushes on rectum as it descends.  If anything does come out, your doc will casually sweep it away.

Depends: Hopefully you mean you had pyelonephritis earlier in pregnancy. If so, it should have no impact at all during delivery. It is a kidney infection treated with antibiotics. Once it has resolved then no special treatments or precautions need to be done during the actual delivery. If you are in labor now with a current kidney infection then antibiotics will need to be continued.

Need Exam: The skin of the perineum should be somewhat flexible aloowing you to have sex without tearing or splitting. When that does occur, it could just be that you are unlucky and it is normal for you. But it could also indicate a more significant problem with that tissue. You should see your gynecologist for an exam and testing as necessary to define the problem. Whatever is going on is likely treatable.

Not uncommon: Blood incompatibility between mom and baby may lead to increased levels of jaundice during the newborn period. Incompatibility is due to antibodies within the mother's blood, which may be secondary to a mismatch between blood types (mom is type o and the baby is a or b, or mom is rh- and baby is rh+), or the presence of an unusual antibody, which is typically screened for during pregnancy.

Option 2: pain medication given as an injection or intravenous.
Option 3: epidural-an injection given in the back which takes away the pain from the waist down. While still allowing you to move around freely in the labor bed.
