Ear blockage?: if the ear is painful when you tug on the lobe then you could have an outside ear infection - you will need to see someone for rx.  If it is blocked from wax - you could try to put some hydrogen peroxide in it and see if that unblocks it - but if that doesn't work may need to let someone look into it

There is nothing: about PSP (Progressive supranuclear palsy) that would preclude the ability to perform ear cleaning and unblockage nor that would preclude the use of analgesic medications for pain. You need to take her to a health care professional to flush/clean her ears and prescribe pain medications.

Possible: more commonly ear aches prior to any headaches...trying to use a qtip to "pry it out" sometimes only impacts it more and causes ear and headaches....
seek medical advise should this continue

Eustachian tube bloc: Yes.  Inflammation from viral or bacterial URI will do it.

Hearing Difficulty: Sounds like earwax (called cerumen). The reason sound is muffled is because sound waves cannot pass easily through solid objects including wax.  You can try mixing 5 mL of tapwater and 1 mL over the counter hundred and peroxide.  Use an ear dropper and put several drops into the affected ear.  As the solution dries it will harden the wax and it should come out.  NO OBJECTS IN EAR!

Try: Putting alcohol on the ears after swimming to fully dry them out . Then when they feel better you can buy over the counter drops for swimmers ears to use that will help on a maintenance basis

how effective is pseudoephedrine in fixing ear pressure blockage during flights?

It may work: It may work.. It just takes a while and you may not need it:try performing a valsalva maneuver - pinch your nose and gently "blow" air out of your nose while at the same time bearing down like you are lifting something. This will force air up your eustachian tube and expand the middle ear and prevent traction on the ear drum which is what causes the pain and fullness during descent.
