Stomach anatomy: Anatomically, the peri-gastric abdomen includes: spleen, pancreatic tail, stomach, descending duodenum, splenic flexure of the colon, associated vessels. Don't forget the diaphragm, heart, &amp; lower chest are pretty close by. The abdominal musculature, ribs, &amp; skin can all contribute as well to localizing peri-gastric "stinging" pain.

If abdominal pain : Becomes sharp or severe, you'd need to take him to the er. Swelling of the spleen is common. Don't keep pushing on his abdomen.  One of the rare complications of mono is  rupture of the spleen. That's why he can't play basketball, football, or any other contact sport for 2 mos. After the acute 10-day illness is over. The liver can swell also. If he develops jaundice, take him to his doctor.

Skin infection : Description matches a localized infection. It could be an early abscess. In any case you have to see a medical practitioner to get it tested with a swab culture, and  treated with incision and drainage, and/or antibiotics. In the meantime put warm soaks or compress to help it get to a head. It can spontaneously drain.

Insect sting : If there is a stinger, it may be removed with forceps. The swelling may represent a large insect bite reaction. If so, apply calamine, antihistamine lotion and 1% hydrocortisone to the area. If there is an increase in swelling, redness or fever, please see a physician right away.
