Possible pituitary : I am assuming your post-stimulation cortisol was also low. If so, some possibilities are use of corticosteroids (prednisone, hydrocortisone for example as well as cortisone shots in joints or for an epidural), chronic pain medication use and pituitary dysfunction. Having said that, it is unusual for there to be isolated ACTH deficiency- usually there are other pituitary hormones that are deficient

Obesity: and insulin resistance, possibly PCOS are the most common, but you need to discuss w/your doctor. Ask about possible use of metformin to lower insulin resistance and spironolactone to lower testosterone. Plus you need diet, exercise and weight loss.

Exercise acth test: I do not believe that exercise 3 hours before an acth stimulation test will affect results.

being diagnosed with hypokpp but now just got low cortisol reading of 4.2, did cort stim test today no results yet. could this be secondary problem?

Probably normal: Hi. What time of day was your cortisol of 4.2? It's probably normal. Low serum potassium (K) may be a result of too much aldosterone or too much cortisol, but adrenal insufficiency (primary) causes HIGH serum K, NOT low. Your doc may be barking up the wrong tree testing you for primary adrenal insufficiency. Good luck!

Yes: If your saliva cortisol tests are low, then you have adrenal insufficiency.  Acth stim test will only help determine if the cause is in the adrenal glands, pituitary gland or hypothalamus. Without other hormonal abnormalities or abnormal levels of sodium or potassium, this test is generally not needed and treatment for the adrenal insufficiency can be done.

Unclear question: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Having said that, F,S and Z are types of alpha 1 antitrypsin. If the total level of the protein is normal, you do not need to worry about these types.
