SJS: A very serious skin reaction with very high morbidity and poor prognosis. Drugs are the most common cause, but viral infections (including hiv) have also been implicated. If there is even the slightest concern for sjs, you need to seek immediate dermatologic evaluation as this is a medical emergency. This is in the same family as erythema multiforme and toxic epidermal necrolysis.

Erythema Multiforme: Erythema Multiforme (EM minor) is a type of allergic reaction that occurs via a distinct and separate mechanism from immediate hypersensitivity reactions (hives).  The rash is longer lasting, and generally does not respond to antihistamines.  A more severe version called Stevens-Johnson Syndrome (aka EM major) also affects mucous membranes (eyes, mouth, genitals) and requires immediate assessment.

Replacing hormones.: Sheehan's syndrome is a rare condition also known as pituitary apoplexy/ischemic stroke in cases of severe postpartum hemorrhage. If the acute event results in chronic hypopituitarism, then adequate hormone replacement and close care by an adult endocrinologist are warranted to avoid severe or even life-threatening complications. Prevention is paramount, with proper perinatal care.

Unsure: What is the harm? If it doesn't cause any harm, I wouldn't worry about it. Has he been through some trauma? Sometimes, people need something to help calm them, and decrease anxiety. Perhaps he just likes having something under his arm- some people sleep with a pillow there. In the long run, I don't think it's a problem unless *he's* concerned or it prevents him from interacting with others. .

Act now: Inaction will absolutely result in child removal and likely incarceration on your part. So any action has to be better. Make immediate change to separate the two--stay w/other family, lock door, whatever. Talk to his dr now for psychiatric referral/eval and treatment to stop recurrence. Talk to her dr for referral for psych therapy to be sure no long-term ill effects. Get dr's help now.
