It depends: With today's improved composite fillings and with modern polishing techniques, these fillings really tend not to discolor, even over many years.  If you are a heavy coffee or tea drinker, or smoke, it increases the chances of staining or discoloring.  I have seen these fillings stain in a year and have also seen some of my fillings last 15 years without any breakdown or discoloration.

Yes: A dentisti could replace a discolored filling.  Better him than an md.  But...Keep in mind when you replace a filling you to take away a little more tooth structure and you could end up with a much larger filling that could then lead to a crown or other restoration.  Your dentist will let you know.

Replacing a filling-: While all that is necessary is to remove the existing composite filling and not any tooth structure, not only is it difficult to be exact, but often we want to reach new enamel for the best bond, and may even want to refine the preparation slightly.  The additional amount is negligible.

Cut it our: It sounds like a silver-mercury tattoo. Aka amalgam tattoo. Only cutting it out and possible gum transplant graft can fix that.

The : The realism is actually up to the esthetic ability of your dentist.  The white filling will blend into your tooth, if the right shade is selected.  It is up to your dentist to shape it in a realistic manner.
The filling will stain just like you're natural teeth and can be removed with proper hygiene.

Staining: The tooth colored composite fillings used years ago were more porous and therefore picked up stains easier over time. The newer materials can receive a higher polish and maintain their integrity for longer periods of time.

Be powerful: Is your smile so take responsibility for it in always. Amalgam should not be placed is there a better materials available.  Gold is also being replaced with new ultrahard porcelains.  New techniques have made a world of difference in both function and cosmetics.  Disfiguring materials are unnecessary, and you should be respected in your requests.  Finding a different dentist is a choice.

can fillings cause discoloration on my gums? i had a silver filling used to fill a cavity, and now the gum next to it looks like it is turning bluish or something. i don't know what's going on. should i see my dentist?

Yes : Yes you should see your dentist to make sure that the discoloration is related to the silver filling that was placed and not being caused by some other problems.

If during the procedure to place the new filling the dentist had to cut our an old filling something called an amalgam tattoo can occur.  This happens when a fine dust of silver impregnates the gum tissue giving it a blue grey color.  Like any other tattoo the mark is permanent unless surgically removed.

do i need a third opinion if one dentist tells me i have 2 tiny cavities that need to be filled and another dentist tells me that one of those 2 tiny cavities is just a stain on the tooth, while the other cavity might need work due to an old filling?

: You can get a third opinion but don't be surprised if you get a completely different opinion. Each practitioner may diagnose you differently. How about saving a lot of time and restore the one tooth that needs a restoration and watch the other one every six months?
