An exam: It would be unusual to have an internal papilloma, more likely it is an internal style, or chalaza on. However have an exam by an ophthalmologist, either way, it canbe removed.

: A biopsy after treatment is recommended. If no treatment has started, than an oncologist needs to be consulted for possible treatment. Radiation or surgery or what is best will be discussed. Don't wait, cells can be cancerous and stop smoking if you do so.

Hpv: There are meds that affect the manifestations of the virus (ie, the papillomas) but it takes the patient's body to get rid of the virus.  Imiquimod is an immune modulator that is very effective.  Both tri- and bi-choroacetic acid can destroy visible lesions also but have no affect on the virus.

my 3 year old has had a squamous papilloma for almost two years now inside his lower lip. how did he get this? no one in the family has hpv 6 or 11.

No certain answer.: Oral squamous papilloma are thought to be caused by HPV, most commonly  6 and 11, it is possible that other strains could also cause the lesion.  In addition, unless it has been biopsied, squamous papilloma could be confused with verrucas vulgaris which is also thought to be HPV induced, but with strains that often cause common warts on the skin but can occur on mucous membranes.

Skin cancer: An inverted papilloma is a kind of skin tumor in which the abnormal epithelium grows down and in instead of out on the surface.  Dysplasia means some of the growth has abnormal features, on the way toward but not all the way to cancer.  Squamous cell carcinoma in situ means the dysplasia is severe, and is essentially cancer confined to the epithelium.  Invasive carcinoma is full-blown cancer.

Premalignant: An inverted papilloma is a tumor in which surface epithelial cells grow downward into the underlying supportive tissue. It may occur in the nose and/or sinuses or in the urinary tract (bladder, renal pelvis, ureter, urethra).. With dysplasia and in-situ squamous Ca this would probably be a  premalignant sinus carcinoma bordering on squamous cell Ca needing resection.

: It may be one of the many nonsexually transmitted HPV types, as are common hand and foot warts. Or maybe sexually transmitted: all sexually active persons acquire HPV, and as you already understand, HPV can be dormant for many years and then reactivate. If your doctor is an otolaryngologist or oral medicine specialist, s/he probably is very knowledgeable and you can trust the no follow-up advice.

: HPV is the likely cause of the squamous papilloma. Although HPV is very contagious, some of the genetic sub-types are slow growing and may have co-existed "for years" without achieving a size where obvious, and biopsy is necessary.
