Rash: Rash between buttocks is usually ' jock itch' or fungus. Treat with antifungal and steroid ointment for about a mnth

Eczema?: This can be due to eczema, a fungal rash, or a contact allergy rash. If this bothers you, you can try some over-the-counter Hydrocortisone cream, but you might want to see your doctor for a definitive diagnosis.

Pilonidal cyst: This is a common place to find pilonidal cysts which can cause Sx as you describe.  If this is recurrent may want to see your doc to consider treatment. On the other hand this could just have been a bruise. Sometimes you don't remember getting hit....

Fecal soiling: Whenever it feels  wet or irritated, use some skin lotion on bathroom paper or kleenex. Using your finger as a guide, clean the area. Clean inside the anus. After a bowel movement clean the area the same way. After a big fart clean the same way.

Needs evaluation: With this sort of painful rash - I would recommend showing it to a doctor. That way you can get an accurate diagnosis and start the right treatment for what you have. I wonder if there was any 'risk' you might have engaged in to cause this? The answer probably lies in the history. This needs treatment though because you don't want it getting any worse- particularly in a sensitive area like this.

An appointment to see md is important to look for other diagnoses is important.
