The injury implys a twisting form of trauma.I suppose a kid who stuck his leg in a hole & fell over could do it, but the possibility of inflicted trauma must be considered.

Nonunion: An xray is needed to see if the bone healed  if not healed that could be causing the pain. If bone is healed pain could be coming from soft tissues or from the metal. Needs evaluation.

Unlikely: Usually initial degree of displacement is the worst. Should be ok in cast. Needs to have serial xrays. Needs moderate displacement for surgery in a 12 year old.

Yes: However, with a variety of details missing, such as preexistent conditions, anatomic details of the fracture, previous functional status, etc you will likely need physical therapy due to a degree of deconditioning, muscular atrophy. Being young and keeping a positive outlook are helping.
