Chemically/hormonaly: A biochemical reaction occurs when the first sperm reaches the zona pellucida of the ovum that prohibits other sperm from entering. That first sperm subsequently sheds its cover and its dna is expanded inside the ovum so that the combination of the two haploid dnas can take place and create a brand new dna identity!

Two ways: Ivf is fertilization outside body. We do not use test tube but rather plastic dishes. We sometimes simply place the washed sperm cells in the dish with the egg (conventional insemination). Sometimes the sperm is the fertility problem and we can insert a single sperm directly into the egg using a microscope and tiny glass needle (icsi - intracytoplasmic sperm injection). Only about 25% -> embryos.

Could be: Either one, but most likely not either: it's still probably an embryo problem. You don't say 'what' tests were done, but karyotype only identifies large chromosomal abnormalities, and histopath tells you nothing much at all. 98% of these are genetic abnormalities, but one smaller scale.

Depends: Could be either.  A blighted ovum is fertilized egg that is not developing properly.  It is not concerning unless happens 3-5 times. If this is your first pregnancy, don't worry too much.  Stress can effect your body.  Focus on being healthy, eating well, resting, folic acid and give your body 3 months to recover.

Unlikely : Several follicles containing eggs begin mature each cycle. One usually becomes the dominate follicle and ovulates. The others simply atrophy and go away. That sounds like what is happening to you. In the case of identical twins one egg is fertilized and then divides to produce 2 embryos. This could still happen. Hope this helps.

Unlikely an issue: Cpx is not known to target the ovary so ovum defects if present  would be unrelated.There are some reports of an inflammation of the testis with the disease but this is also unlikely to be a factor. Sperm that are altered by any force are not the healthy swimmers that produce a pregnancy. Cpx during pregnancy can produce fetal changes & neonatal cpx detected at birth is life threatening.

Miscarriage: There are many possible reasons for miscarriage.  I would recommend seeing a reproductive endocrinologist for a complete evaluation.
