One concern is whether or not there is low sex hormones. Sometimes, a smaller penis is normal or may run in the family. Chromosomal analysis is sometimes done to see if there is a genetic cause. A pediatric urologist can also help.

Small penis: If everything else about your boyfriend is normal - testicles, hair distribution, height, etc - the cause usually is low testosterone levels in the later part of pregnancy that he was exposed to. It happened a long time ago. There are surgeries to correct this problem, but unfortunately they are not always successful. A urologist could give him whatever options there would be.

See edocrinologist : This sounds like an endocrine problem that needs a good work up and then treatment depending on your needs and goals.  An endocrinologist or Urologist with experience in endocrine problems is your best bet.  You may need to go to Rome or Paris etc to find the specialist.
