Kind of: The fat pads are normal structures and so they don't really heal.  In a x-ray of a normal elbow, they are usually not seen as they are within the joint.  When the elbow joint has extra fluid in it, they float out of the joint and are visible.  So when you see fat pads on an x-ray, that is a sign of a significant elbow injury even if a fracture is not seen, as in an occult radial head fracture.

Knee arthritis of: The wear-&-tear variety (osteoarthritis); patella is kneecap; small collection of fluid (effusion) in the space above the patella. If friend is overweight, encourage weight loss to ease stress on ankles/knees/hips. Tylenol (acetaminophen) or nsaids (aleve/advil/motrin/etc) can relieve discomfort. If effusion gets too large, may need to be drained.

Swelling: An effusion is a medical term for swelling.  The MRI is a sensitive tool that picks up even the slightest abnormality in a given area.  A small effusion is a small amount of swelling in the joint.

This could be due to a number of things, from arthritis to ligament and cartilage injury.

Probably nothing: Small joint effusions can be caused by many, many things, most require no testing or intervention. If the joint hurts, non-steroidal anti-inflammatory medications such as advil (ibuprofen) can be helpful as can ice massage. Mostly rest and time. Imaging is not usually required for a non-traumatic, small effusion.

Likely you will.: That sounds like a severely degenerated knee, especially for someone of your young age.  That youth will likely relegate you to surgical correction - knee replacement - at some point, usually when the knee starts to feel unstable of if the pain becomes so severe that you simply can no longer use the knee effectively.  But why is the knee so bad?
