Check With Doctor: The surgeon (or a nurse in the office) should be able  to answer this. Generally, the doctor will want to see you for follow-up after most any surgery anyway. The surgeon knows what s/he had to do to correct the fistula and can best judge how long it should be until  full healing occurs.

Hemorrhoid?: A hemorrhoid (enlarged vein) seems most likely. Whatever the cause, it doesn't sound serious as long as there's no pain, bleeding, or discharge from your anus. See a doctor for exam and diagnosis you have any of these symptoms or if it continues to concern you. I hope you're taking precautions against HIV and STIs. Good luck.

Vaginal anatomy: There are three main orifices in the female anatomy- the urethra, the vagina and the anus. You can google an image and compare it to what you see on yourself. It sounds as though you have correctly identified that urethra and the anus. If there are two other opening then you should schedule an exam. If you are a virgin you may simply be seeing the hymenal ring.

Hemorrhoids: There are three steps to self treating hemorrhoids (blood clots in the rectal veins).

1.  Soak: keep the area clean so that infection doesn't set in.

2. Soften your stool:  keep from bearing down hard when having a bm so that the clot doesn't grow

3. And finally, steroids to shrink up the inflammation.

Sentinel skin tag: A sentinel skin tag is usually results from a fissure or tear of the anal tissue following passage of a large constipated stool.  The tag itself is not painful, but the fissure can be uncomfortable both with passing stool and at rest, just like any other cut of the skin.  
Increasing fluids and fiber in the diet is usually recommended to help in healing but the skin tag remains.

: This may be entirely normal, especially if there is no pain or other symptoms. But impossible to say with certainty based only on this description. See a doctor to check it out. (Why are you examining your anus if you have no other bothersome symptoms?? Are you maybe at risk a particular problem, such as receiving anal sex?)
