what causes a periumbilical ventral abdominal wall hernia?

Opening in muscle : You may have a weakness where your umbilical cord went thru. Or it can be from a surgery that left a weakness there.

can periumbilical hernia be repaired under local anesthesia? and when driving is possible after the operation?

: Mostly yes. But depends on size etc and the decision by the surgeon.  Driving can start after 24-48 hre.

i am 4 weeks post periumbilical hernia repair with no mesh, and i started feeling swelling & now protruding a bit more since yesterday. the day before i put sheets on my mattress, would it be irritated from that or is it possible for reoccurrence?

Go to surgeon: You need to follow through with a surgeon visit as a peri umbilical hernia indicates that there is usually a loop of intestine underneath that protrusion that can get trapped and it needs to be repaired.

abdominal us shows small supraumbilical hernia mid abdominal line containing only fat ,i'm overweight, what is your opinion and advices as lifestyle changes and others and can this tiny lesion heal by preventive measures with time ? regards?

: Best to get BMI down to 25 and see if it gets better.  Not likely to be a problem.  Esp is you have not symptoms

Suggestions: There are some things you can do to improve your discomfort while you heal. Try the anti-inflammatory naprosyn (naproxen) if you're not taking it already since it's long-acting). I suggest also increasing your water intake a lot (improves blood flow to spasmed tissue), use a microwaveable heat sock to low back several times a day and avoid stretching, as that typically just recreates the mechanism of injury.

Talk with your doc..: Refer this Q to your doctor since the marginal findings of US have to always correlate with clinical profile so to make a reasonable, not perfect,  judgement since no test bears a 100% of accuracy and specificity. Besides, all life events always display on their pattern of full spectrum; therefore, it is hard to make a perfect diagnosis especially at their early almost non-discernable stage.
