: The report of the second study should have directly commented on whether the change is significant and indicates significant enlargement of the cyst. Discuss with your doctor, who in turn may want to have a discussion with the radiologist.

EXPLANATION?: Arachnoid cysts are not uncommon, and involve congenital, lifetime, usually benign and incidental lesions.  These are NOT "brain tumors" and in most cases cause no notable symptoms.  Most radiologist reports reflect accuracy from MRI

: If the cyst is big enough it may compress the brain or spine but it most often does not cause any symptoms. Yes, we can tell the compression when we look at the CT or MRI brain images. Base of the clinical symptoms and the images, we will decide if the cyst needs to be taken care of.

: Tough to compare size on CAT scan vrs MRI.  Since you appear so worried and focused, suggest you see a neurosurgeon for a second opinion regarding the best ways to objectify and manage this arachnoid cyst.

Very likely : Arachnoid cysts are faiily common congenital lesion. We see them all the time.  In general lesions outside the brain are benign, and lesions inside the brain can be more serious.  When lesions are small and in a typical location, it is not usually a problem to recognize them.  IV contrast usually not necessary.  But it could help to detect and characterize other causes of symptoms.
