Avoid dairy: Avoid red meat and dairy, add vitamin c, b complex and omega three fatty acids and you may see an improvement with in a few months while you take your medications.

High cholesterol: My go to drug for high cholesterol (c) reduction is Crestor (rosuvastatin) and it needs to be given in reduced dosage in patients with chronic kidney disease. I would start you on a dose of 5 mg/day. That will likely not be enough to lower your levels significantly. You need to go to an endcrinologist (e) to see if the e can add other drugs you can take to lower your dangerously high c. Good luck.

Can ask one's GP: Many diet pills just decrease one's appetite a bit (pills are often related to stimulants). A person's desire to eat can be much stronger that a diet pill's power to decrease her appetite, so she will likely continue to eat too many calories. Also, diet pills don't increase one's desire to exercise. If something as simple as a diet pill actually worked, we would not have so much obesity in people.

However, most people with kidney damage are placed on certain dietary restrictions such as salt, potassium and fluid intake. Much of this depends on the degree of kidney damage involved.

INR: There are a bunch of things which can cause variation of inr which is why we monitor with at least every two week inrs when the patient is on warfarin. One of the things is also lab error. Lupus anticoag could contribute to an abnormal inr measurement.  As inr of 4 can be associated with excess bleeding should get the situation clarified by your doc.
