If he has a history of sensitivity to medications/substances on his skin, watch for any pain, increased swelling, or redness and contact his doctor if he develops any of that (could be increase in infection or reaction to medication).

Sebaceous cyst: Most do not get infected. Consult with us to get an Rx and send a photo to monitor to be sure it heals without an issue.

Not sinus: No sinuses in that area. Probably a cervical neuralgia or muscle pain from the spine. see a neurologist

See a dermatologist: You may have something called acanthosis nigricans, 
which is really not an infection. If the back of your neck feels velvety and soft and is darker than the rest of your skin it is possible you have this. To be sure i would see a dermatologist, 
 since it can be associated with other things.

Yes: Herpes itself is usually a skin infection. There are many bacteria that can cause skin infection as there are viruses. Fungal infections are also skin infection. It is not always clear to a doc the specific type of organism causing infection. Herpes will often look different when infecting different body parts: fingers, mouth, genitals (different male vs female).
