Penile skin rash: There are many genital/penile skin conditions. Most are not serious. "Red and shiny" suggests lichen planus, especially if he also has white or slightly lavender or purple streaks in his mouth, inside the cheeks. However, "dry and scaly" suggest other kinds of dermatitis, like eczema and many more. He should google both for photos, then see doctor to confirm diagnosis and get the best treatment.

Ant-fungal cream: Sounds like a fungus. Where on your feet is this: on bottom, top or all over. How long have you had this problem. Have your feet been exposed to anything that may have caused a reaction: ie bug bites or a new pair of shoes?

These lesions are pre-malignant and is large enough should be removed. The doctor usually makes this diagnosis by viewing the lesions and then biopsying them. Diffuse actinic keratoses (diffuse AK) is a condition where there are multiple AK lesions on the body.

Fungal infection: Dry scaling feet and "water spots" which I am assuming you little blister are mostly likely caused by a fungal infection. Vesicular tinea pedis is a type of fungal infection that i believe you may have. See a doctor, podiatrist, derm for further evaluation. You can use anti fungal creams but may need pills if it is really severe.
