The conditions are: asplenia (either absent or nonfunctioning); complement deficiency; nephrotic syndrome. Anyone undergoing cochlear implantation is also at higher risk.

It depends: on the type of meningitis. Most teens are vaccinated for meningococcal meningitis and some for hemophilus influenza B and pneumococcal, but if it's viral meningitis or a different bacterial meningitis, they are not protected and could get it. Also the vaccine isn't 100% effective and there is a booster recommended at age 16 (initial one usually done at age 11).

No less serious: There are some other infections or forms of infections that can show similar signs and symptoms as meningitis, but i would not describe any of them as less serious, in fact they are more serious. 
Some of them are encephalitis, leptospirosis, 
subdural empyema (collection of pus between the brainsurface and the inner lining of the skull) and brain abscess.
