Premature beats: Are you sure you only get irregular beats when you're lying down or is it that you're unaware of them when you're active? Wearing an ambulatory(holter) monitor for 24 hours would give you precise information about your heart rhythm. Avoid caffeine, nicotine, chocolate and alcohol if you're having frequent premature beats which make you feel like the heart is skipping. Check for an overactive thyroid.

Arrhythmia : You can have an irregular heart beat or arrhythmia due to many causes. It is unlikely that such a low dose of escitalopram will cause it but your arrhythmia can worsen at any time. When starting a psychiatric treatment, it is best to request an EKG and full blood work to rule out medical causes of psychiatric illness like thyroid disease, which can also cause arrhythmia. Please seek advise.

Avoid triggers: Skipped beats usually imply premature ventricular or atrial contractions (pvcs or pac's).  They are triggered by Adrenalin surges or stimulant effects (caffeine/alcohol/meds can trigger).  Pain/breathing issues/physiologic stress can trigger as well.  In the setting of anormal heart structurally these symptoms are more of a nuisance than a danger.  Void the triggers.

Need more info: It is necessary to know what your heart rhythm is. If you have fibrillation, it sounds as if you are going in and out of that rhythm. You need to have an event or holter monitor, to see what is going on and so your medicine can be adjusted.

Arrhythmia: There are basically atrial and ventricular premature beats. Additionally there are junctional and fascicular extrasystoles which fall under the atrial and ventricular umbrellas. Within the grouping of tachy and bradyarrhythmias there are many types.
