However, Viagra's effect may be limited because of your age and significant comorbidity.

: This is definitely a discussion to have with your nephrologist (kidney specialist).  Some dialysis patients can tolerate low dose viagra.  It depends on other medications, and other health problems you may have, if viagra would be safe and effective.

Blood thinners: Lovenox and Coumadin (warfarin) are started together but once Coumadin (warfarin) is therapeutic, Lovenox is stopped. Plavix can be taken with Coumadin (warfarin) or combo of lovenox/coumadin. The risk of bleeding and the use of different anticoagulants and anti platelet drugs should be carefully decided by the physicians and is done on a case by case basis.

if a patient has an pt of 60.1 and is taking plavix (clopidogrel) for atrial fibrillation, what orders would you take? would you discontinue the plavix (clopidogrel)?

Coagulation defect: With a pt of 60 your risk of bleeding is high. Why you have it is a problem. Plavix (clopidogrel) is an anti platelet agent but at the risk of bleeding you have i would stop it. Your doctor has to talk to you and find out all the medicine you are taking, evaluate your liver and your coagulation system to find out why you have coagulopathy.

Answers: If you have MS and ED, you may have spinal cord involvement as the culprit, so, would first focus on the MS. Get off Avonex, it is the least effective drug on the market, and may be contributing to your ED.  Consider an oral agent such as Gilenya (fingolimod) or Aubagio.  You certainly could take testosterone but there are risks and a urologist should get involved.  Take vitamin D-3 supplements.

No easy answer: Based on BP and how frequent the patient is monitored  and how active the chest pain. The key is drop in BP and that may cause more harm, Also viagra (sildenafil) has a short half-life, so if the patient took if many hours prior, and has a stable BP, NTG is an option, and then there is a hypertensive patient, NTG may be beneficial to get the BP down and decrease workload.
