Hive treatment: In general, it will take a couple of hours since Prednisone needs time to suppress the production of histamine. On the other hand, antihistamine works faster. It suppresses the hives itself. Your doctor would decide which medication works best in your situation.

See a dermatologist: A steroid shot is not a treatment of choice for hives. Due to blood transfusion? Was it really necessary? Do you really have hives, or something else? How long have you had this? Ever before? Using antihistamine? Which one at which dose? Why estrogen AND Premarin (conjugated estrogens) AND Provera??? No wonder you are bleeding.

Hives can be: very RESISTANT to treatment...especially if no SPECIFIC ALLERGY has been identified....

: If the vaccine caused the hives I would have expected them to appear no later then a few days later. I recommend consulting an allergist for evaluation and advice about taking any future booster doses,

Antihistamines help: Your medicines from the er may have worn off. If they gave you prescriptions you should get them filled. Antihistamines such as Diphenhydramine (benadryl) or Cetirizine (zyrtec) should be taken continuously until your regular doctor can evaluate you. Be cautious of sleepiness with the use of around the clock antihistamines.

Probably allergic to: something that is still in his system.  So initial symptoms were controlled by medicines but came back when they wore off.  Was he given any oral medications to take, or told what to do if symptoms came back?  If not, see your doc or return to the ED.  Emergently if any breathing difficulty.

Possible: Allergy shots may be associated with a late-phase reaction which comes on several hours after (local reaction). But it is highly unusual to develop hives 2-3 days later. I suspect that something like a viral infection may be the cause for your hives.
