Care of kids: The best arrangement is to plan ahead.  Make arrangements for your children with grandparents or very close family friends. Remember that labor can begin at any hour and most people lead busy lives.  You'll need someone who has either has a very flexible schedule, or is not very busy to suddenly take care if your kids.

Not proven: There is one study that proposes, at the molecular level that an excess of  oxytocin during birth could cross the infants blood brain barrier in sheep and mice.
This was published almost 10 years ago, and as far as i know there has been nothing further. On the other hand there are some studies that are proposing the use of oxytocin to aid in repetitive behaviors seen in the autistic spectrum.

Varies: Progesterone is a great medication proven to reduce pretty labor risk. If by injection, injections are usually given weekly. Or some use daily suppositories of progesterone. Most continue the meds until 36-37 weeks. After last dose of injection, it is assumed to work for 7 days. Suppositories effects are done when they stop use. Most say a week week period of working is realistic after last dose.

Pelvic : Pelvic pressure warrants a medical evaluation. You may be dealing with something as simple as a urinary tract infection (uti). On the other hand you may have uterine fibroids (benign tumors of the uterus) or even ovarian cysts. Also vaginal infections can present this way. 
As you see there are a few different possibilities. Your doctor will need to do some testing and will also likely order a pelvic ultrasound. When all the results are in, your doctor will be able to tell you if there are any issues that would/are intefering with your ability to conceive, and will aslo give treatment as deemed necessary.

Pregnancy: Birth control pills after a pregnancy is to prevent another pregnancy. If you had complications your doctor might not want you ( and you as well) to get pregnant too soon.

High Risk: Yes , you may have an incompetent os which has been responsible for your miscarriages.

Labor: When the contractions are painful and five minutes apart for at least an hour it is time to be seen in labor and delivery. If you feel your pain is severe you should be promptly evaluated. If you are having trouble breathing then you should be seen in the ER. Over the counter medications such as Omeprazole are safe for use for heartburn.

Impossible to predic: Unfortunately - there isn't a good way to predict labor. If you are already dilating that's possibly a sign -but still not reliable for predicting when labor will start.
