Rib cage pain: Your symptoms sound like costochondritis which is inflammation/irritation of the cartilage the lies between the ribs and near your breast bone (sternum). This is usually treated with over the counter anti-inflammatory medications. If your symptoms persist, I recommend seeing a rheumatologist. I am happy to do a consult if you desire.

started last night sharp pain behind right shoulder blade when breathing worse with deep breath, radiates into right neck and arm-seems better today??

Could be a: pinched nerve in your NECK (C6-7)

If gets worse a cervical exam/neuro eval and possible cervical MRI study may be indicate!

Chest pain: Given your age, it is very likely that you could be experiencing an acute episode of anxiety which doctors commonly refer to as a panic attack.   However, there are other causes of acute chest pain that first need to be ruled out such as heart related pain.  Other causes could be acid reflux and muscle/bone pains.  It is best to seek medical treatment as soon as possible for your complaint.

Need chest xray: The pain with deep breathing suggests pleuritic pain (due to inflammation or fluid between the lung and chest wall).  The fever and dry cough suggest infection.  The pain in shoulder could be due to pain from fluid/inflammation involving the diaphragm known as "referred pain."  a chest xray and/or ct scan of chest would be helpful to determine if there is fluid outside or inside the lung.

You like spiders!: Anyway I can just interact directly?  Brown recluse usually causes severe necrosis in the area of the bite.  There is nothing to do for these bites unfortunately until weeks after the bite.  The pain and necrosis all occur at the site of the bite.  So if you were bit under the armpit, then yes, could be related.  Otherwise, no.

Maybe: Difficult to answer this question without an examination and localization. There are nerves, lymph nodes, muscles, tendons, etc. in that general area. Please see your local provider.

That's often tender: area normally. The teres major and minor (side muscles of the chest) are normally tender by the ribs, and is not a problem. If your pectoralis major, minor (breast muscles) are tight, these become even more pronounced. Usually, we are not pressing ourselves there much, so it's not noticeable or bothersome. If it's PAINFUL, do see a doctor soon!

Get reevaluated: There are a number of things that could be giving you these symptoms.  Make sure you see your orthopedic surgeon to determine exactly what is going on - then you can determent the correct treatment. Continue with your physical therapy, this can be very helpful if you are patient.
