Thoughts: Scrotal swelling in a young male is not at all normal and needs to be looked at quickly. Possibilities include, cancer, epidydimitis, testicular torsion or a hernia. Please seek help as this is something that can be treated if caught early.

my right testicle becomes horizontal sometimes and when the scortal sac is loose then the testicle comes back to normal position. there is pain in the groin only sometimes. i am 28 and i have never had this condition from past 27 years.started lately?

Horizontal testicle: I would follow up with  urologist as I am concerned that do to the movement you could develop testicular torsion which is a medical emergency and not good for the testes. A urologist could tack it down and decreased movement. Call a urologist today

Here are some...: Cure is impossible, but just modified to heal and optimize functional recovery. If indeed having epididymitis &amp; antibiotics is effective, epididymitis-related pain &amp; swelling will start to fade in 2-4 days after antibiotics, but the resolving pace for all its related symptoms may linger for weeks &amp; even months. After pain disappearance, scrotal puffiness/swelling may represents water in the sac.

For asthma med?: I presume you meant the spacer for asthma medication delivery.  It is usually somewhat loose so that you can clean the inside. Just push the parts back together since the fitting does not need to be very tight to do the job.
