Old infection: would be the most common explanation, but there are some other illnesses that could cause this and be more worrisome.  You need to ask the physician who ordered the x-ray what s/he was worried about, and what is the cause for these findings.  Good luck!

May not be a  problm: Why was the study  done? What these  show depend on yo clinical picture and may not be a  probem. You need to discuss with your  physician. If necessary can get them biopsied.

Vague: It is a very vague description that basically says the lymph nodes look abnormal in some way, but it's minimally abnormal and there is no pattern to the abnormality. Best to discuss with your doctor but usually, clinically this doesn't really mean translate into anything. (i.e. non-diagnostic, not really helpful for guiding treatment either) Hope this helps.

Reactive=monitoring: Lymphatic system absorbs/carries things too big to go into the arteries/veins like bacteria eaten by white cells and digested food. Lymph nodes "taste" the lymph for anything bad  like bacteria/viruses/cancers, etc. When it detects a problem, it alerts the immune system and your body reacts.  They enlarge in response to the inflammation they create when they react.  They shrink when all is well.

Not too concerning: Lymph nodes can normally range in size from 1cm to 1.5 cm. Unless they are getting bigger on serial imaging studies and grow up to 2 CM, there is no need to be worried.Of course what I have stated has to be taken in the context of your medical history which you should provide us before you can receive a reliable opinion. Why was the CT scan of the Mediastinum done? We need history of any symptoms?

Usual: There are many there.
If tender, relates to some inflammation in the area, or sometimes a viral illness like mono.
If non tender and enlarging , some other concerns.
Supraclavicular can relate to pulmonary or intra thoracic issue.
See your pcp and perhaps some x-rays and occasionally even a biopsy if indicated.

Lymph nodes in your: There are many reasons for the neck lymph nodes to get enlarged. Most often the cause is infection or inflammation of your throat. Such nodes are often tender to touch or pressure. You can watch the size of the lymph nodes. if they get any bigger, then an examination by your doctor is advised. A biopsy of the node may be done if there is any suspicion of lymph gland cancer(called a lymphoma).

Just what it says: Lymph nodes, located where they are supposed to be, visible on the CT scan and at a size that is generally considered normal. Taken out of context, it is impossible to tell you anymore than this. But even in the context of some known disease, these likely mean nothing.
