: You should have it directly evaluated today, if possible.  You may have viral orchitis requiring anti-inflammatories, bacterial epididymitis requiring antibiotics, and whatever’s causing the pain/swelling might be making your testicle more likely to twist (called torsion) in a way that limits its own blood supply.

If : If he is having abdominal and back pain, and decreased urination, as well as testicle pain, then he needs to go to the er immediately.  He could have a kidney or bladder infection, kidney stone, epididymitis,  or testicular torsion.  He could also have an abdominal process such as appendicitis, especially if he has having any vomiting or fever.  Please take him immediately in for evaluation.

Depends: It is not uncommon to have discomfort after an inguinal hernia repair for several weeks. In fact you probably won't feel completely back to normal for 6 to 8 weeks.  If the pain is getting worse or not getting better at this point you should be seen by your surgeon for follow-up evaluation.  If there is no redness or drainage and the pain is getting better over time it may be  safe to watch.

Black and blue?: Sometimes when people have surgery the blood will descend to the lower level and cause pain. This would be from the time of surgery. If this pain is severe then you need to see the doc to make sure everything is ok with the surgery. Check with surgeon as soon as possible. The stinging is not unusual.

Epididymitis?: This could be epididymitis, which is inflammation of the epididymis. It can be caused by trauma or infection and can brew for several weeks. When you were looked at earlier, it could have still been early enough to not be caught. Worth a trip back to the doc for re-evaluation and possible treatment. In the meantime, a bag of frozen peas works wonders for testicular pain.

Urologist: This symptom is concerning and should be evaluated by a urologist. He will carefully palpate the testicles and order an ultrasound which is usually quite helpful in ruling in or out a testicular malignancy.

Yes indeed: Internal pelvic pain, originating in the bladder, prostate, etc is a kind of &quot;visceral&quot; pain -- the same kind that occurs with intestinal or other problems inside the abdomen. Such pain is always quite diffuse and can be perceived in places quite far from the origin. Also, pain can radiate to areas served by the same nerves. Prostate or bladder pain often radiates to the testicles.
