Hard to say : Would need more info and exam.  If it has been fairly recent swollen nodes can cause similar symptoms and are often associated with a viral or bacterial infection.  A good medical checkup would probably be a good place to start.

i woke up at 4am with very sharp right shoulder pain. it is still hurting especially when i take a breath.

Be seen: Many possible causes, but usually chest and/or shoulder pain that is worse with inspiration would be well suited to be urgently evaluated just to rule out a cardiac or pulmonary cause.

Couple of things: Could be a couple of things. If not going away in a couple of days, then should have it checked by someone. Maybe even get X-ray.

Maybe lung related: Sounds more like lung issue. Check with your medical doctor to make sure your don't have pneumonia. You also need to see an orthopedist to try to isolate exactly where the problem is

Not clear: Typically if pain is related to movement it is muscle related. You need to also consider pneumonia or in your age range to consider, though unlikely, a blood clot especially if you are on birth control. Heart disease or inflammatory diseases are something to consider although less likely.   See your doctor for a full exam and history.

Family Practice: Shoulder pain is common with gallbladder disease. Even after surgical removal of your gallbladder the pain can persists. Treat it symptomatically but call your surgeon to let him know how you are feeling.

i had a surgery for a hiatal hurnia almost two years ago. and i am still having shoulder pain when i breath deeply. what could be causing that?

Follow up: I would certainly recommend followup to discuss with your surgeon.  It is either somehow related to the surgery/hernia, or it may be that you have another condition causing your symptoms that has not yet been diagnosed.
