All sounds normal: At 18, probably your genital skin is naturally maturing. That often includes prominent natural skin structures:  hair folllicles, sweat glands, and sebaceous glands (also called fordyce spots) all become more prominent with sexual maturity. Pearly penile papules also can show up -- they're normal too.  "Growths" under tougue may be papillae near salivary gland ducts. See doctor if remain concerned

Without a direct look at glans penis and further review of detailed clinical Hx, it's hard to decide if red spots on glans penis is long tight foreskin-related or not. If yes, circumcision is advised for recurrent balanoposthtis after ruling out other possibilities such as psoriasis, std-related skin disorders, etc. So, seek pro-evaluation &amp; counseling timely.

: HIV doesn't do this. Usually it means balanitis, especially in uncircumcised men. Most balanitis is "nonspecific" and clears up with penile hygiene -- retract foreskin and wash with soap and water a couple times a day. A mild steroid or anti-yeast cream often helps. If it persists despite these tries, see a doctor, ideally a urologist. Good luck.

Skin infection: Red, pain, small dots, large red swelling. It really doesn't matter. You have something wrong with your penis right now and it is not wise to wait this out or to nit-pick if one does not have all of the possible symptoms.  Your doctor needs to see this and to get you treated.

Possible infection: You should go see your doctor and get examined.  It may be an infection that would require a prescription medication.  If it has been present for 6 months it is not getting any better which is even a stronger reason to get a professional opinion.
