Dermatitis...: It could be a dermatitis which is an irritation of the skin after it comes in contact with anything that causes an reaction on it. You may use hydrocortisone over the counter but if it is painful or gets worse you should see your doctor for exam and treatment as needed.

Herpes zoster?: If all blsiter-like lesions and rash are strictly on one side of the body's midline, then the best bets are either herpes zoster (shingles) or recurrent herpes simplex, probably type 2 (HSV2). Bacteria like staph or strep, allergies, and other dermatitis also are possible. (My bet is shingles.) See doctor ASAP:  if either shingles or herpes, effective Rx must be started immediately. Maybe pilonidal cyst: A pilonidal cyst is often the culprit of these symptoms.  There is a cystic area under the skin that often becomes infected and drains spontaneously.  They can be very painful.  He should see his primary care doctor for referral to a surgeon for excision of the cyst, which should take care of the problem.

Perhaps : Perhaps consider using a zinc oxide ointment twice daily.  It could be numerous things including psoriasis, eczema, skin allergy, fungus, or yeast. Desitin (zinc oxide diaper cream): Treat it like a diaper rash and use a thick cream like desitin- keep a thick layer on at all times until it goes away

Infection?: It sounds like you may have a peri-anal abscess, or a rectal fistula, or maybe a chronic anal fissure. All can be be serious or can become so. If you have had anal sex, then an STD like gonorrhea or chlamydia is possible. You need to see a doctor soon. Good luck.
