: It has been 5 days with pills and cream treatment as well. It may not scab. If the skin is all intact and no open wound from the popping, you can consider it healed.

?Canker sores: Canker sores are not contagious. They may look like a pale or yellow ulcer with a red outer ring. You may have one, or a group of them. Women seem to get them more than men. The cause of canker sores is not clear. It may be due to:

a weakness in your immune system for example, from the cold or flu , stress or
lack of vitamins and minerals in the diet, including vitamin B12 or folate (folic acid).

Because this lesion has been present for weeks, you should see your child's doctor soon for correct diagnosis and treatment.

Likely oral herpes: You probably have oral herpes virus infection. It will go away by itself, but may recur if you have fever, or get exposed to bright sunlight.

Mucosal warts: Warts do occur on mucosal surfaces like the lip. And one of the changes that the virus induces is a change in the blood vessels, so it could be red. More often the epithelial changes predominate however and it appears a bit whiter than the surrounds. Still if it persists you should have it seen as there are other things that can cause red raised bumps too.

Yes: irritant from soaps or other contacts are possible, but that doesn't usually cause a solitary painful red bump.  Frankly, herpes is the first that comes to mind, but there are lots of possibilities.  Get checked out if it doesn't go away in a couple of days.

Swollen Bump: The red bump could be a bacterial infection in which case you would need antibiotics. The bump could be a herpes infection in which case you would need antiviral medication. See your doctor for the proper treatment.

If it hurts...: ...It's probably a localized infection in a sweat gland or oil gland. If it doesn't hurt, it's probably an angioma, which is a small collection of blood vessels; so don't squeeze it! if it's the latter, it will bleed like stink. Localized infections usually go away; angiomas don't, but treatment is easy with laser or electrocautery.
