Hidradenitis: You may have boils (furunculosis) or hidradenitis suppurativa.  Mrsa, a bacteria is also a possibility.  You need to see a dermatologist for accurate diagnosis and to start treatment.  I usually use a cleanser such as Phisohex daily, a topical antibiotic twice a day and an oral antibiotic.

Hidradenitis: I would speak to your doctor about your general health.  Are you diabetic?  Are you overweight?  People with hidradenitis supurativa get "pimples" in their groin and buttock and it is not related to weight or diabetes.  I find topical antibiotics and/or fruit acid treatments help. I would see your family doctor or a dermatologist.

Folliculitis: You should consult a physician to get treated . My guess is folliculitis which may be due to staph infection and it requires prescrption antibiotics.

Wound care: Likely wound care with a small amount of debridement. Rarely would a patient in that age group qualify for flap surgery to close the wound.

Perirectal abscess?: You need to have a doctor examine the skin area and treat this appropriately. An abscess is a chronic skin infection that will not improve unless opened with an incision surgically, drained of pus, and treated with antibiotics. A perirectal abscess can be serious if left untreated, may spread and cause septicemia- please have your GP evaluate the area and decide on next steps, immediately

Rotation: Must change point of maximum wt on skin by changing patients position every 2-4 hours, easier said than done in this case however, he is lucky you care.

That : That brings up a question.  Unless you are bedridden why do you have two large open sores? Are they both in the midline, above the crack of the buttocks? If so, that might suggest pilonidal cysts. If they are on the cheeks and really large, that brings up questions like infection, immune issue or as stated earlier if you are bedriddent the skin could break down. I recommend that you be medically evaluated to determine the cause.
