Get seen: Nothing you can do -- topical treatments, hygiene, diet or lifestyle -- will help much with bad acne. Your personal physician has a host of prescription-strength Rx's. If those around you are superstition and are interfering, pick up the phone yourself. If you are offered isotretinoin, remember it's not for the faint-hearted by the risk-benefit ratio is strongly in your favor. Best wishes.

Get seen: At your age, perhaps there is an explanation for why you have severe acne, or perhaps it is something else. Or it may be time for a different approach to your treatment. If you are using something that works to suppress your acne, and stop, it will recur unless you have finally outgrown it. I was able to go off tetracycline around age 50. Good luck.

Laser or Accutane: You can settle for over the counter topical creams that dry your skin and give temporary relief. But then you also need to buy a whole lot of cream to cover your shoulders and back. A better solution would be a series of laser procedures to clear the acne away. The most effective and permanent treatment, however, is Accutane (retinoid). Given in gradual doses it works wonders, on the back/ face.

None works: No "home remedy" touches acne. Benzoyl peroxide is the only over-the-counter treatment that works. If your acne is bad, it's a crime not to get you treated -- growing up is hard enough.  All acne is easy to manage with today's rx's; please get with a savvy physician. Good luck.

Dermatologist helps: Acne is caused by skin pores being plugged by tiny clumps of skin cells (the clump is called a comedone). When there are no bumps or actual pimples and pustules, the acne-prone areas of skin will still have plugged pores. Maybe the black spots are comedones in the skin pores. Dark spots can also be post-inflammatory hyperpigmentation that takes months or years to fade after pimples break and heal.

Seek cause: Something as simple as increasing his strength using a bench press can get this started. I'm assuming no new medications or habits, especially not lithium. Many of us men simply have a testosterone surge from time to time. Severe acne usually warrants a systemic antibiotic plus a topical such as Clindamycin with a retinoic acid derivative or benzoyl peroxide. Should be sufficient.

You should see a: dermatologist if you're not already. They may be able to help you more (or get a 2nd opinion from another one). There's not much we can do to help on this site. Sometimes there are stronger medications that can be used, but you would have to be seen in person.

: Rub skin area with honey or apple cider vinegar .Drink green tea or rub extract on skin. Apply witch hazel or moisturize with aloe vera.

Wrong approach: Acne is a chronic, non-lifestyle-related disease that's a grave personal and professional liability. Nothing you can do at home will clear severe acne. You need a physician's help. The medicines cost relatively little except for isotretinoin, but even if this is necessary, it's cheaper than not being able to get a job because of your face. Do what you need to.

Dermatologist: In this day and age, there are so many treatments available,  Topicals, oral medications, even laser therapies.  You need to review this with a dermatologist to come up for a plan best suited to your skin and the type of acne you have.  Best of luck.
