why can't i sleep without tv on, else my mind race through random thoughts, is this normal?

Sleep: Yes you are normal.  Many people use the tv, radio or music to put themselves to sleep, as opposed to many who prefer a quiet dark room.

lately eachday when alone i have intrusive very random thoughts,(like those if falling asleep).stops when very focused/distracted. what could this be?

Need specifics : I would say you need to provide more specifics in order for us to answer that question. Depends on what those intrusive random thoughts are. Could be Focus issues, obsessions, a thought disorder. Speak to a professional.

i have a lot of internal dialogue that tends to be very negative, like random thoughts saying something negative about someone. what are some useful cbt tools, or other types, to change those automatic thoughts from coming in so frequently?

: I recommend seeing a mental health professional who will coach you how to channel negative thoughts, turn them into positive ones and  also to ward them off.

: It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor and discuss if you may be depressed. Counter-intuitive as it may seem, exercise may help. Start with low intensity exercise such as walking and gradually increase the intensity as tolerated. Drink more water. Wish you good health!

for several years i sometimes get debilitating cramps during sex. prev dr never found cause. 8 mo after baby, its back. so random. thoughts?

Cramps during sex: I had a patient that had cramps after sex that were due to the prostaglandins in the ejaculate.  Her pain was better taking advil (ibuprofen) an hour before sex.  There could also be an organic cause, so i would see a gyn to rule that out.  Also if you do not have these cramps when pregnant, then a birth control pill may do the trick.  I hope this helps you some.  These are just thoughts.

See Doctor: Nervous system concerns are very important. Epilepsy comes to mind , tumor, hypoglycemia, anxiety and others. See a doctor for an exam, labs and possible referral to Neurology and Radiology for imaging tests. Don't wait do it today.

Paranoia: Please see your doc to rule out medical conditions.  Then consider seeing a mental health professional to help you w/what may be obsessive thoughts and mild paranoia. Chest pain: Highly unlikely that a single cup of coffee or tea is related to your chest pain. Also unlikely that this is cardiac in nature given your age.

when i close my eyes and so tired (trying to sleep) suddenly random thoughts come to my mind like a very fast dream.should i worry?

Tired &amp; thoughts: When we are drifting into and out of sleep the filters for our thoughts are less strong and random and strange thoughts can float through our mind.  This MAY be what is happening to you.  Check w/your doctor, especially if the thoughts are negative or dangerous.  Peace and good health.
