: The explanation should be done by the doctor who has followed you for this electrolyte imbalance. I have seen patients with the same parameters but the etiology (reason &amp; explanation) has to be individualized. E.g. what types of vitamin D were measured because a patient with low D3 but the active D3 can be high or when calcium is normalized it takes time for the PTH level to normalize, etc.

I wonder if you have diabetes insipidus (not diabetes with sugar problems) that makes you lose excessive amounts of water. If you have diabetes insipidus you would be urinating more than 4 liters a day, often thirsty and have a tendency towards dehydration.

Low vitamin D: Can lead to higher PTH levels, which should normalize w/d replacement. If not, need to look for other causes such as a parathyroid adenoma or benign tumor of the parathyroid gland. Also make sure she is not receiving hctz (hydrochlorothiazide) diuretic, which can increase pth. Calcium needs to be checked, and if BUN is still high, she may need better hydration. Check w/doctor about her renal status.

Primary hyperparathy: If you have a high calcium and high PTH level, you most likely have primary hyperparathyroidism.This is a relatively common condition, especially in women after menopause. It has nothing to do with your diabetes.  In your age group, this would be treated by removing the abnormal parathyroid (likely 1 of 4 parathyroid glands is abnormal). Without treatment it can lead to kidney stones and bone loss.

PTH should be low: If you calcium (and ionized calcium) is high, your PTH should be suppressed to low levels unless you have a parathyroid adenoma secreting excess pth. Supplementing low vitamin d is important. Also make sure you are not on thiazide diuretics. Your endocrinologist can evaluate the cause and treatment. Don't ignore it.

Speak with your doc: It sounds as if your doctor is doing a diagnostic work up to include your parathyroic gland.  If this is the case she may also order tests to visualize the gland as well as your bones.  You may have had a recent fracture or problem that she is addressing. No worry: In early stages of hypothyroidism TSH is high when T3 (liothyronine) T4 may still be normal. Hypothyroidism is quite common and is easily treatable with an oral medication. However, you must follow your doctor's advice for additional tests and treatments.
