Chondromalac/compart: When knees develop early tendonitis change, there can be periods of flare where inflammation and pain can develop behind the kneecap due to cartilage tears. This is also called patellar chondromalacia.There is also concern for compartment syndrome given your vascular complaints. It is thus best to see a Sports Medicine &amp;/or Vascular MD to diagnose what is going on and make sure of the diagnosis.

Inflammation: If there's a pressure build up inside your knee, it will exercise pressure on the knee cap. Usually fluid build up will do so. That, in its turn, is a result of inflammation inside of your knee.

Fluid under knee cap: It may be hard for you to know whether you have fluid under knee cap or not. Your doc, however, can do certain tests in the office that will help him determine that.

KNEE PAIN:  knee pain and popping you need an exam baseline labs and knee X-ray .  It may be chondromalacia patellae.(Runners knee) If  duration of knee pain is over 3 months  a knee MRI would be indicated to define the anatomy and evaluate for structural damage. Also consult a PMR doctor for potential injections of platelets or glucose . Lastly an ortho consult in case surgery is needed

Knee impact injury: As long as the skin is closed, I would ice the area, elevate, and limit movement for a little while.  Over-the-counter medications such as Tylenol (acetaminophen) and/or Advil would be beneficial.  If the pain does not subside within a day or so, or if there is any pain with weightbearing, I would see a local emergency department or urgent care and get an x-ray  to make sure that he didn't break the knee cap.

Needs to be checked: It may be a cyst and since it hurts to touch,it better be checked by your doctor.Needs to be examined and may need xrays or MRI etc. to make a diagnosis

Quad muscle: This sounds like the vastus lateralis muscle which is one of the quadriceps muscles. This can be painful with a strain injury to the muscle or tendon injury called a tendinosis if it has been going on for a while.

Patellofemoral dys.: What you are experiencing is probably due to either prepatellar bursitis (some swelling on the outside of the patella) or inflammation of the back of the patella where is contacts the groove in the femur. A physician can help sort this out for you.
