Confusing post: A hole in the heart is a common birth defect but not one I think of from Accutane exposure. If you want to have your kid assessed for this or other exposure related problems seek an evaluation by a medical geneticist, often affiliated with a medical school or children's hospital. They would go through both parents family tree and any findings that may indicate a source or recurrence risk.

Pregnancy unlikely: Using a reliable birth control method correctly all the time, works very well. However, life can surprise you sometimes. If a woman is not getting her birth control pill withdrawal bleeding during the inactive pills, or if she is just worrying about pregnancy rather often, she can do a home urine pregnancy test once in a while. Tests are cheap, very reliable, and sold in stores everywhere.

First : First of alll let me advise you not to do your own cervical exams, it's best if you are pregnant to leave that to your doctor. To answer your question, it is very difficult to say if you will miscarry or not, however you should see a doctor for an evaluation. If your cervix is found to be incompetent (weak) then your doctor may place something called a cerclage (stitch on your cervix) this keeps it closed, however this may require you to be on bed rest.

7% pill/29% spermici: Effectiveness with contraception is reported as either the lowest expected rate (l) ( with motivated patients who are reminded - in a clinical trial, for ex.) or the typical rate (t)- seen in a general sampling of the community - at random.  The pill ranges from 0.3% (l) to 8% (t) and spermicide ranges from 18% (l) to 29% (t).

: You continue to be protected if you change pill brands, even if you changed ingredients and dosages somewhat. Occasionally the change will trigger breakthrough bleeding for a cycle or two, even when they are supposedly exactly the same.
