You can but ...: This is referred to as placentophagy and has been part of many different cultures in the past - and even present.  But like consuming any "meat" it can transfer blood/tissue borne infections and must be prepared and stored in a fashion to prevent spoilage. If the placenta is covering the cervix then a C-section is needed for delivery.

Good question: In general, great than 4 cm.  It's thought to be due to various intrauterine infections and increased fetal morbidity and mortality.  The literature is all over the place, however.  If baby is growing fine, that's all that's important.

Random chance: The embryo implants on the uterine lining, and at that location the placenta starts to grow. Usually, it is on a portion of the uterus that will stretch and thin during pregnancy, and is nowhere near the internal cervical opening.

However, sometimes the  embryo implants very near the internal cervical opening, resulting in a low-lying placenta or even a placenta covering the opening, req. C-sec.
