Worry about infectio: You may have cord compression, epidural abscess, discitis or osteomyelitis of your spine. Would recommend going to ER for blood cultures x 3 sets (separate times and separate sites), CBC, cmp, esr, crp and spine imaging and a thorough physical exam by a physician (not midlevel). Make sure they do not give you antibiotics before doing the blood cultures.

See a doctor: There are far too many causes of skin rash, including those that involve the genital area, to even guess at the cause in this case. The rash/itching may be unrelated to sweating at night, and none of it has anything to do with being trans. FYI, the large majority of genital skin rashes are not herpes or any other STD. See a doctor, ideally a dermatologist.

Normal.: There are sweat glands located all over the body in varying amounts.  Some of us rarely sweat on arms and legs, when we are sweating in other areas.  Where you sweat does not determine where you lose fat. There is no relation.  Keep exercising and count calories.

Sweats on legs, DM: Can be related to low glucose at night , would have a pre-bedtime snack ( apple 1/2 sandwich) check glucose before bedtime and occ in middle of night  and see if getting hypoglycemic.. May also be related to thyroid-  or autonomic dysregulation  due to neuropathy of diabetes or menopause, medications or rare infections or masses. F/u with doctor.
