i have developed a perineal hernia, after an apr 3 years ago. are there things i shouldn't do?

Live your life: I would suggest you live your life as normal as possible. Wearing some sort of perineal support may be helpful. These are very difficult hernias to deal with, and you would need a hernia specialist if you wanted this repaired. i had a perineal hernia repaired 10 days ago the drain removed 3 days ago the drain is discharging slightly red fluid 20mm after dressing 2 daily  ?

It could be nomal: It could most definitely be normal. I would say that drainage could last for a few weeks. Red flags: increased pain, fever, redness, purulent (yellowish/pus, etc) discharge, etc.

If you don't feel comfortable, make sure you see your surgeon asap.

i have a temporary ileostomy from a total colectomy, how common are hematomas and infections or other problems in the reversal surgery site?

Ileostomy reversal: Although it varies slightly depending on exactly what kind of reversal will be done, surgical complications such as infection and hematoma are rather unusual these days. If you follow your surgeon's advice, they are unlikely to happen. If you have any problems after the surgery, be sure to contact the surgeon promptly. Good luck

which is best after colectomy: ileostomy or j-pouch?

Depends: This is highly dependent on why this surgery is indicated. If the person is not too elderly or have other risk factors such as crohn's disease or extensive cancer, then a j-pouch may be appropriate. This is performed by a trained surgeon who has the proper knowledge. Some patients do not have a choice due to their disease condition and will need an ileostomy.

Depends on the: Indications for the surgery. Crohn's disease should never be treated with a j pouch. Then it is an issue with the patients preferences and life style and skills of your surgeon. J pouch usually requires a temporary ileostomy and thus an extra operation to close the temporary ileostomy. These are issues to discuss with the surgeon and contact your local ostomy association, meetings and ask members.

if subtotal colectomy with creation of brooke ileostomy placement has been performed, would the rectal stump still be present or is it  removed?

It is often: preserved if a REHOOK-UP is the plan!

if someone has had subtotal colectomy w/ileostomy placement, can a partial colectomy be performed later on down the road?  and what's the difference?

Big difference : A subtotal colectomy is removing almost all the colon; usually just a couple of inches are left near the rectum.  A partial colectomy is not done AFTER a subtotal colectomy because there is basically no colon left. A partial colectomy implies that only a segment of colon is removed, like the right or left, and a good amount of colon is left.

Ileoproctostomy: Ileoproctostomy can be done after ileostomy provided the particular patieint is a candidate for the takedown and anastomosis of the ileum and the rectum.

what parts would a patient need to have in order to be a candidate for ileostomy reversal  after having had subtotal colectomy w/ileostomy placement?

Rectum: As long as at least some of the rectum is there. the possibility for ileostomy reversal exists. Ask you surgeon about your specific case. If your not satisfied with the answer, consider another opinion from a board certified colorectal surgeon. Hope this helps!

if patient had total colectomy with ileostomy placement, can they have a surgical procedure involving ileum to sigmoid/left colon several yrs later.

YES: if one performs a left colectomy, even extending the dissection below peritoneal reflection, as long as there is a rectal stump, 1-2 cm above the levators, proximal bowel can be reanastomosed.   One can creates a pouch from the proximal bowel to make anastomosis easier and have the pouch act as a partial  reservoir for the liquid feces in the ileum that will be used for anastomosis
