Hemmorhoids: The most common reason to have thickening in the perirectal region would be hemorrhoids. These are swollen veins that sometimes develop clots in them. An infection in the peri-rectal region can cause swelling, such as herpes, warts, and bacterial infections. A perirectal abscess can also cause swelling and pain. If there is significant pain and swelling, you should be seen.

Need surgical eval: After three weeks an abscess should not be draining any longer. You probably have developed a fistula which is a tunnel connecting the anus to the skin, it requires an operation to fix it

Both possibilities: Certainly chronic disease can cause perirectal abscess. We also know the Remicade (infliximab) increases risk for infection. Without knowing everything about your case i would suspect you always had crohn's. Your physician should be able to help you clarify that question.

: Yes, the study is consistent with severe constipation causing  bowel distension and inflammation that will definitely cause pain and referred pain. Other causes, related or not to the described condition cannot be ruled out.

Not really: I see this fairly often, in fact.  Hemorrhoids can cause pain, but it's usually an anal fissure that causes the long term pain like you are describing.  It takes a visit to a surgeon to diagnose it.   Either is easily treated.
