Depends: Hcg is the "trigger" shot for ovulation.  It is used primarily to time ovulation in fertility treatment cycles.  There are a small minority of patients either treated or untreated who have an ovulation defect in their own triggering mechanism (the lh surge) for ovulation.  In these patients HCG would be helpful as well.  Otherwise most women trigger their own ovulation without difficulty.

Insulin incr. weight: Insulin makes you gain weight.  Insulin gets sugar in the cells which makes you gain weight... in lament terms. Therefore, although you are doing a great job at controlling your blood sugar, you need to still work with diet and exercise to decrease Insulin resistance and lose that belly fat and overall weight.

Not typical RX: Although Botox can be used for nearly any muscle spasm, I am not aware of Botox being used for this purpose.

Varies from practice: The cost of prp is set by individual practices/physicians. It typically is not covered by insurance companies, who deem it "experimental." my practice bills $550 for a prp injection, which includes the venipuncture, the  processing of the blood, the injection, ultrasound-guidance and the office visit. I've seen some practices charge as much as $2000, but most charge $500-1000 per injection.

Initial supervision: The places where vitamin B12 injections can be given, are the muscles of the outer part of the upper thigh, the muscles of the upper part of the buttocks, the deltoid muscle in the upper outer  arm, the ventro-gluteal muscle in the outer part of the hip. Initial supervision by a medical professional familiar with administering injections is mandatory, until becoming comfortable giving injections.
