Probably neither: With a diagnosis of pcos she probably had a cycle where she didn't ovulate. If there's any concern of a pregnancy then check a blood pregnancy test.

If severe pain: persists, you should see your doctor or urgent care center.  Ovarian cysts, pelvic infection, endometriosis, are possibilities.

i have pcos, for 6 years after coming off the pill.  for 3 years now my period is regular.  but my pcos cysts is causing daily pain, so my doctor is putting me back on the pill. a good idea? and why?

Ovary Suppression: Assuming that the pain is from cysts, the bcp works by suppression follicle (cyst) formation. It might help with cysts. Pco cysts usually are very small and not a source of pain, so perhaps the pco is not the issue. Best wishes.

 Polycystic ovary syndrome (PCOS) is the most common cause of infertility in women, frequently becomes manifest during adolescence, and is primarily characterized by ovulatory dysfunction and hyperandrogenism in addition to the cysts.

Usually not.: Cysts  from polycystic ovarian syndrome are usually very small no larger than 6 to 8 to 10 mm in size. If there are cysts present that are much larger then these at times need to be drained or remove before in vitro fertilization.

Surgical indications: Indications vary.If the medical management of PCOS is not successful, then and only then indications for ovarian surgery will usually be considered based  on your symptoms (severity of pelvic-abdominal pains, suspected torsion or hemorrhage...) , reliability of sonographic evaluation (suspicion of ovarian neoplasm...) "Size of the cysts"alone may not be a reliable or clear criteria for surgery.

Not PCOS: Pcos should not cause pain like that unless you are forming very large cysts all the time. That is unusual. You may have uterine fibroids or endometriosis or even interstitial cystitis. A simple evaluation can help to figure things out.
