Rectal Pain: Many reasons could cause rectal pain such as hemorrhoids or anal fissures. You should see a gastroenterologist for a formal exam and possible anoscopy (endoscopic) evaluation

Anal fisssure: Anal  fissure is a small tear in  the lining of anus causes severe pain and some bright red bleeding during and after bowel movements.
In most cases the tear heals in 4-6 weeks on its own.
If it persists despite RX may require surgery.
Other Rx
Keep stools soft
Increase Fiber 25-35 gms/day
Plenty of fluids
Sometimes may need to add NTG gel, Nifedepine gel , and topical lidocaine

See GI: This is not normal and should be checked out. In general this will require an endoscope exam of the colon which is best done by a GI specialist.

Constipation: First, constipation by itself is not a specific sign of colon cancer & colon cancer is very unlikely at age 21. Drink more water. Eat much less solid food until the constipation resolves. Can try suppository or enema too. Try stronger laxative like magnesium citrate. Avoid this problem by eating right, avoid junk food & excess alcohol, eat more fruits & vegetables, add metamucil. See your doctor.

Endometriosis: Endometriosis is possible if it is located between the vagina and rectum which is the most common gravitational location for the same. If you have had children in the past vaginally, have your MD exam and check for a incomplete rectal repair if  there was a significant tear( 3rd or 4th degree) at previous childbirth.

Several things: Some benign causes could include hemorrhoids or breaks in the skin called fissures, while more dangerous cause may include ulcers or even cancer. The should be evaluated by your physician and if you are over the age of 50 you should be considered for a screening colon exam.
